Transition Services

It is the mission of CCSD “…prepare every student to graduate with a 21st century education to be globally competitive for their chosen college and/or career path.” 

As described by the Transition Alliance of South Carolina:“Transition means change, evolution, and movement. Students, parents, and practitioners must work as a team to prepare for the changes that will occur when students move from school to post-school life. As part of that process, it is necessary to know a student’s present level of functioning and where their skills, abilities, and interests lie. This information can be obtained through any number of formal and informal assessments.”

Transition services for students with disabilities include planning with parents, students, school-based teams, and community stakeholders in order to develop plans to facilitate students’ success after their K-12 school experience.
