Transition Services

It is the mission of CCSD “…prepare every student to graduate with a 21st century education to be globally competitive for their chosen college and/or career path.” 

As described by the Transition Alliance of South Carolina:“Transition means change, evolution, and movement. Students, parents, and practitioners must work as a team to prepare for the changes that will occur when students move from school to post-school life. As part of that process, it is necessary to know a student’s present level of functioning and where their skills, abilities, and interests lie. This information can be obtained through any number of formal and informal assessments.” https://transitionalliancesc.org

Transition services for students with disabilities include planning with parents, students, school-based teams, and community stakeholders in order to develop plans to facilitate students’ success after their K-12 school experience.
